Blue Cross and Blue Shield of Illinois has awarded Access Community Health Network (ACCESS) the highest rating for asthma care among its network of independent practice associations. Along with this Blue Star for Asthma Care, ACCESS also earned a Blue Star for Cardiovascular Management.
This brings the total of Blue Cross and Blue Shield of Illinois Blue Stars earned by ACCESS to six; all in vital areas that affect community members. ACCESS has received the stars—based on its latest quality audits—for diabetes, asthma care, childhood immunizations, blood pressure control, cardiovascular management and mental health follow-up.
ACCESS, which is the nation’s largest network of community health centers with 51 locations throughout the Chicagoland area, is committed to creating a “medical home” that offers seamless continuity of care for its patients.
